If you are a new patient seeing Dr. Walker, please download our REGISTRATION AND HEALTH HISTORY forms to bring with you to your first appointment. Having this paperwork already in hand and filled out will greatly reduce the amount of time you will spend in office before you can be seen.
New Patient – Audio

If you are a new patient seeing Dr. Long only, please download the REGISTRATION and the appropriate AUDIO CASE HISTORY form to bring with you to your first appointment. If your child is 14 years of age or older, please fill out an adult history.
